HIPAA Notice of Privacy Practices

Our pledge regarding your health information

We understand that medical information about you and your health is personal. We are committed to protecting it and complying with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws.

Treatment

We use and share PHI to coordinate and provide medical transportation — for example, sharing your pickup/drop-off, mobility level, and clinical notes with the dispatched crew and receiving facility.

Payment

We use and share PHI to bill and collect payment for services — for example, with your insurance plan, Medicare, or Medicaid.

Health care operations

We may use PHI for quality assessment, crew training, compliance, and other internal operations.

As required or permitted by law

We may disclose PHI when required by law — for example, to public health authorities, law enforcement, or in response to a valid court order or subpoena.

Your rights

• Access. You may request a copy of your PHI in our records.
• Amendment. You may ask us to correct PHI you believe is inaccurate.
• Accounting. You may request a list of certain disclosures we have made.
• Restriction. You may request restrictions on how we use or share your PHI.
• Confidential communications. You may ask us to contact you in a specific way.
• Paper copy. You may request a paper copy of this notice at any time.
• Complaint. You may file a complaint with us or with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.

Our responsibilities

• We are required by law to maintain the privacy and security of your PHI.
• We must notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
• We will follow the duties and privacy practices described in this notice and provide you a copy.
• We will not use or share your information other than as described here unless you tell us we can, in writing. You may change your mind at any time.

Changes to this notice

We may change this notice from time to time. Changes apply to all PHI we maintain. The new notice will be posted on this page and available at our office.
